Apply for one or both, it's your choice.

U.S. Passport Book

Valid for travel by air, sea or land.

Validity:

10 years for adults

5 years for minors (under age 16)

Size: 5" x 3.5" (unopened)

Cost for first time applicants:

$100 for adults

$85 for minors

Cost for U.S. Passport book renewal:

$75 for adults

U.S. Passport Card

Not for travel by air. Valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean ONLY.

Validity:

10 years for adults

$45 for adults

$35 for minors

Cost for U.S. Passport Book holders:

$20 for adults

Required Documents:
ALL APPLICANTS MUST APPEAR IN PERSON, INCLUDING MINORS

Adults (Ages 18 & over):

  • Two passport photos
  • Original Long-Form Birth Certificate (with raised seal) OR old Passport OR Naturalization Certificate
  • Valid Driver's License
  • Proof of name change (Marriage Certificate, court order for name change or divorce decree)

Minors (Ages 16 - 17):

  • You must sign your application here at WWML
  • Two passport photos
  • Original Long-Form Birth Certificate (with raised seal) OR old Passport OR Naturalization Certificate OR adoption decree
  • Bring your own photo ID OR Bring at least 1 parent/legal guardian to identify you
  • Submit previous passport, if applicable

Minors (Age 15 and Under):

  • Parents or legal guardians with Valid Driver's License must sign minor's application here at WWML
  • Two passport photos
  • Original Long-Form Birth Certificate (with raised seal) OR old Passport OR Naturalization Certificate OR adoption decree
  • Submit previous passport, if applicable
  • If only one parent applies, submit notorized consent letter from the other parent

** Fee to WILLOUGHBY WALLACE MEMORIAL LIBRARY is CASH ONLY.  WWML no longer takes Passport photos. We recommend Walgreens.
